Access Bus.
You may have seen these around - they are operated by local councils all across the Country and are a door to door wheelchair accessible bus that you ring up and 'book' much like you would a taxi. The service is free to bus pass holders (elderly and disabled) and if necessary a companion and in some areas it's a daily service, in other more rural areas it is perhaps only once a week but nevertheless it is an invaluable service.
